  • Chronic pain

  • Corneal injury

  • Over-utilization of services

  • Under-utilization of services

  • Inappropriate level of care

  • Use of inappropriate medical services

  • Ineffective treatment

  • Permanent/temporary alterations in functioning

  • Lost work time

  • Medical/psychological complications

  • Lack of knowledge regarding disease/injury process

  • Lack of resolution/commitment to treatment plan

  • Lack of established, goal-oriented treatment plan

  • Post-traumatic stress syndrome

  • Complex regional pain syndrome / reflex sympathetic dystrophy

  • Hospitalization

  • Extended temporary disability

  • Excessive medications

  • Other complicating medical factors/conditions such as diabetes, morbid obesity, etc.
